Medical Forms (REQUIRED FOR ALL STUDENT-ATHLETES EACH SCHOOL YEAR)
All student-athletes who intend on participating in intercollegiate athletics at Goshen College must complete medical forms every year. These are completed online using SportsWare Online.
What you need to know:

All student-athletes will need a Physical Exam dated between May 1, 2025Â and August 1, 2025.

Mission of Goshen College Sports Medicine
The mission of Goshen College Sports Medicine is to provide quality medical care to the student-athletes within the athletics department. The success of Goshen College's athletic teams depends on the safety and well-being of its student-athletes. The Sports Medicine staff is committed to promoting a safe and healthy environment for all student-athletes in their intercollegiate participation at Goshen College. Goshen College Sports Medicine shall provide injury prevention, care, and rehabilitation services of recognized excellence.
"The Certified Athletic Trainer (ATC ) works with physicians and other medical personnel, employers, patients, parents, guardians, and athletic personnel in the development and coordination of efficient and responsive health care delivery systems. Athletic trainers are integral members of the health care team in secondary schools, colleges and universities, professional sports programs, sports medicine clinics, corporate/industrial, and other health care settings."
